Case Management
Perfect Health is able to contract with companies or health care clinics to provide case management services. Our trained providers are familiar with services to support with housing, food insecurity, transportation, access to mental health, substance use programs and a range of other common needs.
Behavioral Health Care
Short term and long term support can be offered to care for patients struggling with a wide variety of emotional needs. Perfect Health behavioral health providers are trained to provide brief, solution focused cognitive behavioral therapies, as well as long term, psychodynamic care. These services can be provided at the offices of Perfect Health, or at the offices of the contacted partner.
