Initial Assessment
All clients receive an initial consultation and evaluation. The initial evaluation is a personalized review of current needs, access to transportation, community supports, quality of life assessment, caregiver access, psychological health and wellness, and formulation of a treatment plan.
The treatment plan will eventually be developed based on the assessment with specific interventions and carefully selected resources. The resources and treatment plan is developed in partnership with the client and loved ones and aims to address short as well as long term objectives.
Memory Management
With aging can often come memory loss. However, there are ways of helping people cope with memory loss and also ways of slowing its impact. Perfect Health.net works with clients to help assess their memory capacities and take steps to address immediate and long-term impact. Utilizing cutting-edge technologies and providing hands-on learning opportunities for seniors is critical to making a positive and lasting impact. Some simple yet vital skills and habits can be developed and tracked to monitor improvements and declines in memory.
Committed To Safety
A critical aspect of client care is safety. The providers at Perfect Health.net monitor patient's progress and can be empowered to support the client and their family if safety is an issue with regard to memory, physical wellbeing, or emotional stability.
Psychotherapy for Wellness
Perfect Health.net focuses on emotional support as a primary intervention. Studies have found that seniors are more likely than any other demographic group to face depression due to a large part of their medical condition or physical impairments. Roughly 80% of those over 65 years of age have at least one chronic health condition, and 50% have two or more chronic conditions. Physical as well as emotional illness amplifies a senior's challenges and can lead to compounding medical conditions.
Recognizing the Symptoms
Through psychotherapy support, clients are treated and monitored for mental health symptoms. Licensed care providers travel to the homes, apartments or meet in the professional offices of Perfect health.net to provide emotional help. This is a confidential relationship aimed at addressing the emotional needs of the client.
Integration of Care
Services will be provided in concert with the client's other treatments to ensure continuity of care. Where indicated, medical treatments, medication, physical rehabilitation, specialist, and primary care physicians will be included in communications with Perfect Health.net providers and staff. Collateral work with providers will be HIPPA compliant and dependent upon client release of authorization. When authorization is obtained, Perfect Health.net providers keep the multiple service teams informed of client needs and provide ongoing updates for a best practice model.
Pricing Support
Perfect Health.net was established to support seniors with a variety of needs. Perfect Health.net staff members are enrolled in a number of insurance networks, including but not limited to Medicare, Medical, Blue Shield of California, and Value Options.
We Accept Most Insurances
Most insurances cover all or most of the cost of services provided by Perfect Health.net licensed staff. Prior to your first appointment, Perfect Health.net will assess your eligibility for coverage based on your insurance. Insurance deductibles and co-payments may be required at the time of service. Direct fee payment for services is also an option if a client wishes to pay without utilizing their health insurance.
A sliding scale is available if individuals or families meet the criteria for financial hardship.
Our Process
Initial Assessment: The cost of an initial assessment is $200 though often this cost is covered in part or in whole by health insurance. The initial assessment is provided by a licensed clinical provider who will meet with the client to review the physical, emotional, social, and psychological aspects of their current circumstances. Integration of care is also established at this initial assessment to ensure the client's care team is informed of need and planning.
Ongoing Support: Appointments following the initial assessment cost $150 and include 50 minutes to one hour of direct support. Health Insurances often cover all or part of the ongoing services. Deductibles or co-payments may be required at the time of service. Prefect Health.net staff will review the costs of coverage as well as insurance coverage to determine eligibility, co-payments, and deductible levels.
